Monitoring and evaluation are one of the key program pillars which is aimed at ensuring that program activities are in line with set goals and objectives, timelines are respected and reporting is accurate, timely, and concise. For accurate reporting, the NCD program developed well-detailed registers for Know Your Numbers Screening, and for Diabetes and Hypertension Management/follow-up. A set of reporting indicators (hard and soft copy) were equally developed to track program data to facilitate monitoring, enhance decision making and guide the allocation of resources.

The program also carries out regular (biannual) facilitative supervision visits to various program sites to monitor program activities, identify best practices, challenges and propose ways forward for amelioration in quality of services 

to the beneficiaries. As another means to monitor and evaluate program activities, the program holds monthly Zoom meetings with all program health facilities to build staff capacity, discuss work progress, identify challenges and propose solutions to problems faced at the site level regarding service delivery. During some of the Zoom meetings, various facilities are scheduled to do presentations and share experiences with other team members from different facilities.
